Medical Forms

Charlestown Primary School are trying to become more digital and paperless. As such, lots of our old, paper forms are now online! This means looking after your child has never been quicker or easier. If your child has any of the following:

• Asthma
• Medication that needs giving during school hours
• A broken bone
• Is returning to school, following a medical procedure

Then we need you to fill out one of our online forms. Please read the information below and click on the links to fill out an online form.

Asthma

If your child has Asthma, please complete an Asthma Plan by clicking on: Asthma Plan.

Medication

If your child requires medication to be taken during the school day, please complete the: Parental Agreement for School to Administer Medicine.

Broken Bones:

If your child has a broken bone, and you have been advised by the hospital that they can return to school, this form MUST be completed prior to their return: Broken Bones.

Returning to School Following a Medical Procedure 

If your child has had a medical procedure, please complete this form, prior to their return to school: Returning to School Following a Medical Procedure.

General Medical Conditions:

If your child has a diagnosed medical condition, or is under investigation for one, then please can you complete this form: General Medical Conditions.
